Passport Health Plan
Passport Health Plan (PHP) is Kentucky’s only non-profit, community-based healthcare provider. PHP believes that the community is healthier when we work together!
Because Passport has both a Medicaid plan (Passport Health Plan) and a Medicare plan (Passport Advantage) designs for PHP must fit within the Passport brand guidelines, and also meet specific design specifications for the Medicaid line of business.
Designs for PHP are regularly distributed to 300,000+ individual members, 100,000+ providers, and to advocates and partners throughout the state.
Pieces designed for Passport Health Plan feature photographs representing their members and use of brand colors green and blue. PHP also utilizes the daisy in many pieces because it symbolizes honesty, patience, and simplicity.
